Written email or letter of agreement stating that you and your team members have reviewed our, "Trade Partner" page and will conduct yourself in a manner which is supportive of how we do business with our trade partners as defined on this page.
Certificate of Insurance sent directly from your insurance company with proof of general liability and workers comp insurance.
NOTE: We unfortunately CANNOT accept Workers Comp Exemption Certificates from the state of Florida unless the person(s) who will work on the project site are considered owner(s) of your company and are therefore covered by the exemption.
If your trade is one which is required to be licensed by the state of Florida Department of Business and Professional Regulation (DBPR), you must provide proof of licensing which can simply be an email with your license name and number.
